Jaguar GR.1 XX963 On 25 May 1982 it was shot down near Wesel, W. Germany, by a live AIM-9L Sidewinder fired by Phantom FGR.2 XV422 during a simulated combat exercise. The pilot ejected safely.

There have been a few others
A USAF F-15 fired a sidewinder on another F-15 in Alaska in the 90's but it didnt crash and made it back to base badly damaged
The pilot was transfered to Korea if i remember correctly
An USN F-14 also shot itself down when an AIM-7 launched from the fuselage raked along the bottom of the aircraft.
For anyone who's ever seen the fins of the AIM-7 up close, they are very heavy and very strong.....ripping through the skin of an aircraft is quite easy.
The weapon damaged a large part of the bottom of the fuselage and the crew was forced to eject.
In the first gulf war an Iraqi Mig-29 was observed by US pilots who were engaging them to shootdown his wingman Mig-29 when it passed directly in front of his plane
Iraqi pilots were rumoured to fly with the missle fire button permanently depressed so they didnt miss out on firing on targets
The same Mig-29 was then observed hiting the ground at supersonic speed and the pilot didnt eject

This is what happened, they were taking part in an exercise, supposedly with safe weapons. Apparently in order to score an air-air kill the pilots of the Phantom's would have to squeeze the trigger in order to trigger the gun camera and get footage of the lock through their HUD in order to substantiate their claims to a kill. Unfortunately an armourer had neglected to ensure the missile was safe, so on pulling the trigger to set the camera rolling,the Phantom pilot instead let fly with an AIM-9 that brought down the Jaguar.
